Gender-Affirming Surgery Evaluations and Letters
The Process
1. Schedule an evaluation
To begin, reach out to schedule an appointment focused specifically on your request for gender-affirming surgery. If you are not an existing therapy client, I’ll send the standard intake documentation that is completed by all clients via secure email through SimplePractice. Included will be a brief form designed to give me a deeper understanding of your experiences with transition and surgery goals.
2. Complete the evaluation
During the evaluation, we’ll discuss your experience of gender, goals for surgery, expectations, understanding of the procedure, and relevant mental health and psychosocial considerations. We will also discuss your support system and practical considerations related to surgery and recovery. Throughout the evaluation I will develop a comprehensive understanding of your individual circumstances.
3. Determine next steps
If you meet the applicable criteria, I will move forward with preparing your letter. If additional information or support is needed, we will discuss this and outline the appropriate next steps.
4. Receive your letter
Once it is determined that you meet the applicable criteria, I’ll prepare your supporting letter in accordance with the WPATH Standards of Care, Version 8 (SOC-8) and the documentation requirements provided by your surgeon and/or insurance provider. I’ll ask you to complete a release of information and if you’re able to provide the fax number for your provider’s office, I can fax the letter directly to them. I’ll also send you a digital copy of the completed letter through SimplePractice’s secure email.
*If your surgeon or insurance provider requires two letters, I will connect you with another qualified therapist who can provide an additional, independent evaluation and letter.
5. Continue with your care
A surgery evaluation and letter can be a standalone service. You do not need to become an ongoing therapy client to receive an evaluation or supporting letter. If you are interested in ongoing therapy we can discuss whether working together beyond the evaluation would be a good fit.
